PE make up

Any student absence including family business, illness, medical appointment, etc will count towards chronic absenteeism unless make-up is completed and returned. School business absences are excused. Please inform your teacher the day prior to your school business activity.

Please print and fill out the following form to excuse your absence. You can also find hard copies of the make-up form on the bulletin board outside of the boys' and girls' locker rooms.

Remember, when completing late work you need to complete a make up form. You can find the make up form on the bulletin boards outside of the locker rooms. Parents must sign the form before you turn it in.
